Membership Assistance Program
Regardless of their financial means, Canadians young and old with potentially life-threatening health conditions or special medical needs can be protected with a MedicAlert membership. The Membership Assistance Program provides partial subsidy for the membership fee and the cost of a stainless steel bracelet.
Membership Assistance is made possible though the generous donations of our members and is available to Canadians living with poverty.
How to Apply
Individuals can apply for assistance by completing a Membership Assistance Application Form.
We ask for a referral from the applicant’s physician or social worker, a nurse or a pharmacist. A completed and signed MedicAlert Registration Form detailing contact information and medical conditions should accompany the Membership Assistance Application Form.
Applications are individually reviewed. MedicAlert reserves the right to verify the information provided and consideration will be given annually as to continued assistance.
