When you join the Maureen Roberts Circle of Friends Monthly Giving program, you help us protect the more than 1 in 5 Canadians with a medical condition. This special group of loyal and committed donors to the Canadian MedicAlert Foundation is named after our founder, Dr. Maureen Roberts.

Monthly Giving is:

• Simple – just tell us the amount you wish to give and your donations will be withdrawn automatically from your bank account or charged to your credit card each month.
• Easy – contributing is easy because you make donations in small, easy-to-manage amounts.
• Convenient – no paperwork, no cheques to write, no postage and no envelopes. You receive a single tax receipt for your total year’s contribution.

For MedicAlert, Monthly Giving means:

• Predictability – you help provide us with a steady income to help us budget and plan.
• Year-round Support – your pre-authorized payments support projects and programs throughout the year.
• Less Administrative Costs – you help reduce our costs, so we spend less on administration, paper and postage, which allows us to direct more dollars toward protecting lives.

2.		Mail a void cheque complete with your name and address and a note stating how much you would like to donate each month and your choice of date for processing to the address below. You can choose to have your gift processed on the 1st or 15th of each month. You may send a set of post-dated cheques, instead to: Canadian MedicAlert Foundation 2005 Sheppard Avenue East, Suite 800 Toronto, Ontario  M2J 5B4 You will receive a letter confirming your membership in the Maureen Roberts Circle of Friends
3.		Call 1.800.668.1507 and have your credit card number ready.

Workplace Giving/Matching Gifts

Double your Donation Today!

One of the most effective ways to make your gift go further is to double the gift. Many companies match personal gifts, such as cash, stocks and securities. Some companies offer double or triple matches.

To double your donation:

1. Contact your employer’s Human Resources department to determine if they participate in a Matching Gift program. If they do, obtain the company’s matching gift form and donation guidelines.
2. Fill in the Employee section of the form and send it, with your donation, to the address below.
3. If you donated online, fill in the Employee section of your company’s form and mail it to:

   Toronto, Ontario  M2J 5B4
4. We will complete the form and forward it to your employer’s matching gift office for processing.
5. Your company will send its matching donation directly to the Canadian MedicAlert Foundation.

Tips:

• Companies that do not have a Matching Gift program may match a donation if you ask.
